Printed on parchment.
Binding: Sixteenth-century gold-tooled white calfskin over wooden boards. Leather ties lost. Gilt-edged leaves. Triple gilt fillets form a frame; corner-pieces of a cherub's head and outstretched wings, from which olive branches radiate; the whole decorated with gilt impressions of pinks and pansies; in the centre of the upper cover an oval Assumption of BVM, in the centre of the lower cover, the Crucifixion, each within a floral border, and each flanked by a double addorsed 'C' and an interlocked 'M', monograms of Caterina de' Medici; on the spine a cherub's head, pinks and pansies; see William Salt Brassington, Historic Bindings in the Bodleian Library (London, 1891), 34-5, pl. xvi. Shown in Bodleian exhibition, 1937-8.

On the front endleaf ‘1498'. Bookmark with typographical annotations in red ink and, on rear pastedown, a list of woodcuts in Douce's hand.
Three-line initials are supplied in blue with white decoration, on a red ground with gold decoration; other initials, paragraph marks, and line fillers are supplied in gold on a red or blue ground, with gold pen-work decoration. Capitals touched with yellow wash. Ruling in red ink.
